Reablement is a personalised, short term service which helps you to recover and live independently at home. You may receive reablement following a:
- change in your needs
- loss of confidence
Home First Hospital Discharge Service
Home First is for people who need extra support to get home and live independently, after a stay in hospital. A team of social care experts can support you to recover at home.
After you leave hospital, a member of the Home First Team will visit you at home. They work with you to decide what level of support you need to help you recover at home and remain independent. This is short term support. The team may refer you to Reablement Services for more support if you need it. If you no longer need support, you will be discharged from the Home First Service.
What happens during Reablement?
Staff will work with you in your home to build your confidence and help you to develop the skills you need to stay there. This could include:
- getting around your home
- kitchen skills and support to make meals
- personal care and support to get dressed
At first you will get support for up to 1 week. During this time, we will carry out an assessment with you, working with you to understand your Reablement needs.
If you need more support, we will agree a personalised plan with you. You may need a more time to regain your skills and confidence. We expect that you need less support as time goes on. We will keep reviewing your progress and needs with you.
Who are the Reablement Team?
We are a team of social care experts. We support you to build your confidence and develop your skills to remain independent at home. Our team includes:
Health and Social Care Coordinators carry out an assessment to identify your needs and agree your reablement plan with you. They will review your plan throughout your service.
Health and Social Care Officers work with you in your home. They encourage you to do things for yourself which will help you regain your skills and increase your confidence.
Physical Activity Officers work with you to develop your exercise plan and support.
The Team Leader is the manager who oversees the delivery of your reablement service.
We work closely with other professionals in health and social care, such as Occupational Therapists, Physiotherapists and dieticians. We can arrange additional support when you need it.
Accessing Reablement Services
Speak to Adult Social Care at Newcastle City Council to arrange a referral.
If you're leaving hospital and need reablement. The hospital will arrange with you, so the Home First Team can visit you at home.
Costs and charges
There is no charge for reablement up to a maximum of 6 weeks. To use this service you will need to be willing and able to take part in reablement.
What if I need ongoing support?
If you need ongoing care following your period of reablement, you will be assessed. There may be a charge for this care. For more information please read the Council’s Charging Policy.
If you still need help at home after your reablement service. It will be agreed with you. Your care and support will be transferred to another provider.
What to expect from the Reablement Service
You can expect us to:
- work with you to help you remain as independent as possible
- assess your needs and involve you in developing your Reablement plan. Listen to and include your views and wishes
- make sure you're in control of your care and support
- respect your privacy and dignity
- share information about you with your permission, with other professionals to support your care.
- respect your personal, cultural, social and religious needs
- trained staff who support you with reablement
What we expect from you:
- a safe environment in your home
- tell us of any changes which may affect the support you receive
- you engage with the overall goals of reablement
- to tell us if you are going out or away
- treat our staff with respect
