Rehabilitation, Reablement and Recovery

Help to recover your independence

There are times when you may need support to help you recover after an illness, injury or hospital stay. There are short term support services available to help you to get back on your feet so you can continue to live independently and safely at home.

To use these services you will be referred by a health professional or Adult Social Care. The type of support that you are given will depend on what you need.

Speak to Adult Social CarePoint for a chat about your care and support needs.

Newcastle City Council provides these services:

Reablement: This is a short term service given to you at home after a stay in hospital, a change in your needs or if you have lost confidence for some reason.

Rehabilitation: This helps you to recover after an accident, injury or illness or support you with adjusting to a long-term illness or disability.

Emergency or crisis support. This is short term, live in support for people with dementia or a cognitive impairment, experiencing a crisis or coming out of hospital.
